Tower Health at Home

Tower Health at Home was launched by Tower Health to advance the quality and value of its home health services across the region, expanding its ability to manage the healthcare needs of the community. Building on Tower Healths foundation of high-quality care, Tower Health at Home offers patients a seamless continuum of care following inpatient discharge, as well as ongoing management of chronic conditions, in their own home through a variety of home health services, including skilled nursing care, therapy, hospice, and home care. Tower Health at Home was formed when Home Health Care Management, Inc. (HHCM), the parent company of Berks VNA, Pottstown VNA, and Advantage Home Care, became part of Tower Health on January 1, 2019. At the same time, the Berks branch of Affilia Home Health joined HHCM, and therefore also became part of Tower Health at Home
