My health for life

My health for life is a free, lifestyle change initiative for Queenslanders who are at high risk of developing chronic disease - type 2 diabetes, heart disease and stroke. Delivered in 6 coaching sessions, at a relaxed pace over 18 weeks, the initiative is designed to help individuals discover ways of changing their day-to-day behaviour that they can live with. Its provided in a variety of ways, from groups to phone or online coaching, locally, at home or at work with culturally adapted versions. My health for life has helped over 19,000 Queenslanders to make healthier choices and create healthier habits.
