At the Visiting Nurse Service of New York (VNSNY), our mission is to improve the health and well-being of people through high-quality, cost effective healthcare in the home and community. Weve been a vital part of New Yorkers lives since 1893, with a tradition of vision and action that has taken our industry in new and important directions. On any given day, VNSNY has over 44,300 patients and health plan members in its care. In 2019, we served over 106,000 patients in the five boroughs of New York City, Nassau, Suffolk and Westchester Counties. Our patients range from newborns to the oldest of the elderly (100+), and reflect the socio-economic and cultural diversity of the New York metro area. VNSNY CHOICE Health Plans, our managed care affiliate, shares VNSNYs mission of compassion and care for the poor, the chronically ill and elderly helping them to live safely and independently for as long as possible in their own homes and communities. VNSNY CHOICE coordinates care for its members through its Medicare and Medicaid health plans, CHOICE Total (a Medicaid Advantage Plus Plan), and CHOICE SelectHealth Special Needs Plan for those living with HIV.
